• The USA Hockey Concussion form will need to be printed, signed, and emailed to riverfrontraiders@gmail.com by - this is REQUIRED for play by 10/15/2025

• School and grade verification required by 10/15/2025

Payment Schedule -  Deposit due at time of registration (registration closes 8-15-2025) 

                                     Deposit will be applied toward the total balance due. 

                                     1st payment due 9/15/25   (Senior Deposit Due 250.00-9/15/25)

                                     2nd payment due by 10/15  

                                     Final payment due by 11/15 

*Full payment is received by 9/1/2025 there will be a discount of $100.00 

* Siblings will received a flat $50.00 discount
